In racing at smaller tracks, on rare occassions someone gets hurt bad and the on site EMS will take the person to the ER, but racing is halted til the ambulance returns.

In rarer circumstances, the accident will be away from the track property but the ambulance will still leave and racing pauses just the same.

With that in mind, do DCI shows require EMS people on site? If yes, and they have to leave the stadium with an patient for injury, heart attack, etc., are there DCI policies to pause until the EMS returns? Aside from DCI, what about local or state regs?

if what i understand, and i'm not 100% sure, but there is always a backup on standby to go to the venue if the EMS assigned to the show would have to leave.

At World Championships a few years ago, I remember  a Carolina Crown member hurt his leg severely while in performance. He got medical attention on the sideline, if I recall correctly, and it was nearly immediate. So perhaps there is immediate help at the ready, (maybe moreso in Indy then other shows).

A show promoter here on DCP could probably better answer that question. I do know there is always a security or police presence on site baked into the cost to run any show. I don’t know if that cost includes an ambulance or EMS plan of action as well. Particularly,  for example, a show that may be at a high school with 5 corps.

at the end of the opener, a member of the brass section had a fatal heart attack. EMS was not at the stadium. there was a doctor in the stands who rushed down, and honestly, from all i have heard EMS being there wouldn't have changed the final result, there was an interminable wait to get the gentleman removed from the field. you felt bad sitting in the stands having no choice but to watch, but you felt bad getting up to walk away.

Whoa.

and i remember the Crown member when his leg went in 2010. EMS was there ( plus a volunteer helped get him off the field) but i wish DCI had allowed them to get him fully off before wheeling Bloo out around them

Yes, exactly.  I was there in 2010 as well.  Terrible situation all around.
